Description:
Step 1: Widening the channel to the broken file and flaring the channel.
Use Gates Glidden No. 3 Cutter to extend the channel to the broken file and brush it against the outer wall to widen the channel when the curvature of the channel is less than 15 degrees.
Use the No. 60/.02 conical NiTi file to extend the channel to the broken file and brush it against the outer wall to widen the channel when the curvature of the channel is greater than 15 degrees.
Step 2: Create a 90-degree semi-circular space.
Use E88 (Katana-H) and E89 (Katana-V) to create a thin gap between the inner wall and the broken file. Direct the flat surface of the tip to the broken file from the inner wall and activate ultrasonics by placing it in the space. The depth of the gap should be at least one-third
of the length of the broken file. When the bend is greater than 30 degrees, a drop of medical-grade silicone oil is placed in the canal to facilitate lubrication and loosening of the broken file. The depth of the gap in this condition should be at least half the length of the broken file.
Step 3: Extend the peripheral space from 90 degrees to 180 degrees.
Use E87 (TFRK-S) to widen the semicircular space from 90 degrees to 180 degrees until the broken file is seen ‘dancing’ (moving from the original space to another) under magnification. In clinical practice, the tip is used with pecking and pulsing movements to avoid the accident. Do not press the tip against the channel and activate the ultrasounds because the tip will break due to cyclic fatigue.
To laterally widen the space, the tip should be used in an up/down motion to cut dentin, not laterally.
Step 4
Once the preparation is complete, use the ultrasonic tip used when the broken file was ‘danced’ to perform ultrasonic extraction attempts. First, the channel is filled with 17% EDTA when the curvature of the canal is 30 degrees or less than 30 degrees, or with vegetable oil such as soybean oil when the curvature of the canal is greater than 30 degrees.
Place the ultrasonic tip in the space created in the inner wall and activate the ultrasounds in a short vertical within the prepared space until it is seen coming out of the channel.
